Position Summary
Tideview Terrace, an Eden Alternative Registered Long Term Care Facility located in Digby NS, serving 90 residents and operates on an annual budget of $12M. We are seeking a Registered Nurse to act as the Professional Practice Manager. The Professional Practice Manager is a full time position and part of the senior management team reporting to the Administrator.
This position has primary responsibility for the overall coordination and management oversight of the clinical nursing and allied health programs, including infection control and prevention, ensuring delivery of services is in keeping with the values of Tideview Terrace and meets the appropriate standard for Long Term Care. This position plays a key role in leading and supporting staff in care partnering with Elders.
Role/ Key Responsibilities:
Resident Care Service Delivery:
- Develop and apply best practice standards of care for nursing and allied health programs by revising policy and practices in these areas, including IPAC support
- Ensure appropriate and timely assessments of residents in all areas of functioning, including but not limited to overseeing the InterRAI application through the provincially supported program.
- Ensure appropriate pharmacological approaches are being followed by medication and treatment administration personnel, including but not limited to supporting the eMAR application.
- As Civica Momentum Super User, oversee the clinical indicator data analysis ensuring nurses and allied health professionals are incorporating the use of CAP’s and outcome measures in their care planning work.
- Act as consultant and guide to the nurses to assess resident health and care needs and seek resources for clinical practice skills and development when needed, including pre-admission clients ensuring admissions occur on a timely basis and bed occupancy remains at 99.2%.
- Lead infection prevention and control program and practices, ensuring protocols are in place and followed each day and during outbreaks.
Operations Management:
- Utilize continuous quality improvement principles and evidence-based decision-making processes to ensure delivery of quality care and services to Residents, including but not limited to creation of standard work for areas of responsibility, tracking indicators, seeking quality improvement projects.
- Be the Leader and “Subject Matter Expert” resource to the leadership team and TVT for Nursing and Allied Health Services.
- Liaise with other partners of the Continuing Care Branch to source best practices and best service and address the expectations of Residents and Families.
- Lead and manage implementation, delivery and evaluation of Occupational Therapy, Physiotherapy, Clinical Dietary, and Social Work Services to Residents which includes ensuring individual assessments and programs are in accordance with the needs of Residents and within the strategic priorities, philosophies, policies/procedures of Tideview Terrace and recognized provincial and national standards.
- Liaise with Pharmacists to ensure pharmacy services are appropriate and meet contract standards, including chairing the pharmacy committee meetings.
- Liaise with medical practitioners and LTC medical director to ensure medical care services are appropriate and meet the established standards of care.
- Conduct periodic audit reviews of nursing functions to ensure Quality Management and Risk Management practices related to nursing practices are within standard. Identify areas of non-compliance to other Neighbourhood Managers and Administrator, as required. Support leadership in addressing non-compliance issues, engaging external functional expertise as required.
- Responsible for negotiating and managing all contractual service agreements related to scope of responsibilities with external providers including medical practitioners, pharmacist, chiropodist, cosmetologist, mental health professional, etc.
- Collaborate with colleges and other educational institutions for student placements.
Human Resources Management:
- Work in accordance with respective collective agreements in Tideview Terrace to foster good labour management relations.
- Conduct regular program meetings to ensure information is received within areas of responsibility.
- Coach, mentor, and support employees to establish and maintain good communication practices within and across functional.
- Support orientation, educational training and evaluation for employees, in consultation with Administrator to facilitate continuous improvement opportunities.
Financial Management:
- Assist Administrator with planning and managing budget costs for Tideview Terrace in assigned area of responsibility.
- Research and gather information on costs and best practices in Nursing and Allied Health Services.
- Analyze monthly budget for variances and take measures to improve variances and bring in line with budget. Advise Administrator of any issues.
- Play a lead role in the procurement of medical equipment and ordering of medical supplies.
Qualifications:
- Registered Nurse, licenced in the Province of Nova Scotia
- University Degree in relevant field, preference given to a health care field
- Various Training/ Certification in elder care
- Post professional training in leadership / management
- Prior training as a Certified Eden Associate, Vivage Neighborhood Guide, Person Centered Leadership Model, Art of Hosting, LEAN in LTC, PATH, and InterRAI considered an asset.
- Solid clinical nursing skills from a variety of settings
- Solid communication skills and proven ability to work collaboratively with others
- Service orientated with ability to build relationships and create win/win situations
- Excellent listening, negotiation, team building and problem-solving skills
- Ability to negotiate sensitive conflict resolution situations
- A “big picture” thinker and change agent with a propensity for continuous learning
- Proven initiative researching and implementing best practices which apply nursing standards within a resident-directed care environment.
- Proven ability to manage multiple projects and meet deadlines
- Proven ability to make timely, effective decisions in a fast paced, changing environment
- Knowledge and experience in a LTC environment or health care environment preferred
- Knowledge of LTC Infection Control and Prevention programs and requirements, and training in this field considered an asset
- Solid computer literacy skills including proficiency in MS Suite of applications such as Word, Outlook and Excel, Microsoft 365 and MSTeams, as well as clinical applications including DIS, InterRAI using Momemtum, eMAR
- Flexibility to work non-standard hours if required
- Ability to work a nursing shift on the rare occasion, and take on call / after hours rotations
